Overview:
Astyra is seeking a licensed Clinical Evaluator to join our team in Kitsap County. In this hybrid role, you will conduct in-person PASRR Level II assessments for individuals with mental illness or intellectual disabilities, helping determine appropriate placements and level-of-care recommendations that enhance quality of life. This position offers flexibility, autonomy, and the opportunity to make a meaningful community impact.

Key Responsibilities:

  • Conduct face-to-face PASRR Level II assessments in nursing homes, hospitals, and community settings.
  • Independently schedule and manage assessments in your assigned area.
  • Conduct preadmission screenings to evaluate individuals’ clinical needs and determine appropriate care settings before nursing facility placement.
  • Review and interpret medical records and clinical documentation.
  • Assess psychological, emotional, behavioral, and physical functioning to inform care recommendations.
  • Determine medical necessity and appropriate level of care in accordance with federal and state PASRR regulations.
  • Accurately complete required documentation and submit timely evaluations.
  • Maintain confidentiality and uphold HIPAA compliance at all times.
  • Collaborate with healthcare providers, patients, families, and internal teams to ensure thorough and person-centered evaluations.
  • Participate in required training sessions and internal meetings.
  • The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary from time to time.

Required Qualifications:

  • Active, unrestricted Washington State license in one of the following:
  • LCSW, LICSW, LMSW, SWICL, LMFT, LMHC, RN, or LMHP.
  • Bachelor’s degree in a relevant field.
  • Minimum of 2–3 years of direct clinical experience assessing individuals with mental illness or psychiatric disorders.
  • Demonstrated expertise in clinical assessment, differential diagnosis, and interpreting medical records.
  • Familiarity with PASRR requirements and standards of practice in mental health and long-term care.
  • Highly organized, flexible, and able to work independently.
  • Excellent communication skills and knowledge of Microsoft Office applications, as well as other web-based software
  • Experience working with geriatric populations is preferred.
