As an Independent Clinical Assessor, you will conduct face-to-face assessments for individuals ages 14+ across Clarion, Indiana, and Armstrong Counties, helping drive critical support planning, service eligibility, and long-term care decisions.

Your impact in this role:
• Conduct professional in-person SIS-A assessments
