About the Role

Astyra is seeking a Bilingual Care Coordinator (English/Spanish) to help members access healthcare services, connect with qualified providers, and navigate available community resources.

This is a non-clinical role focused on coordinating care, supporting members through transitions, and removing barriers that may prevent individuals from receiving the services they need. You’ll work closely with members, healthcare providers, and community organizations to arrange appointments, coordinate transportation, and ensure continuity of care.

The ideal candidate is compassionate, organized, and comfortable communicating with diverse populations in both English and Spanish.

What You’ll Do

  • Assist members with navigating healthcare services, provider referrals, and community resources.

  • Conduct non-clinical assessments to understand member needs, preferences, and barriers to accessing care.

  • Coordinate initial provider appointments, follow-up visits, transportation, and other support services.

  • Support members during transitions of care to help maintain continuity of services.

  • Collaborate with healthcare providers, community organizations, and internal teams to coordinate referrals and connect members with appropriate resources.

  • Identify gaps in service delivery and help members overcome challenges related to transportation, provider availability, and access to care.

  • Educate members and families about available healthcare and social support services, empowering them to navigate resources independently.

  • Maintain regular communication with assigned members and appropriate care team representatives.

  • Accurately document member interactions, referrals, appointments, and follow-up activities in electronic systems.

  • Provide culturally responsive support and communicate effectively with English- and Spanish-speaking members.

  • Participate in team meetings, quality improvement initiatives, and ongoing training.

  • Maintain confidentiality and comply with HIPAA regulations, privacy standards, and organizational policies.

What We’re Looking For

Required Qualifications

  • Associate’s degree in a healthcare-related field.

  • Bilingual proficiency in English and Spanish is required, including the ability to communicate effectively with members in both languages.

  • 1–3 years of experience in care coordination, social services, case management, community outreach, or a related field.

  • Understanding of healthcare systems, community resources, and local, state, and federal social service programs.

  • Strong communication, interpersonal, and problem-solving skills.

  • Ability to manage multiple priorities, coordinate services, and maintain accurate documentation.

  • Proficiency in Microsoft Office and the ability to learn electronic case management systems.

  • Ability to work independently while collaborating with interdisciplinary teams.

  • Strong cultural awareness and a commitment to providing respectful, member-centered support.

Preferred Qualifications

  • Experience working with Medicaid, Medicare, or commercial health plans.

  • Case management certification, such as CCM or C-SWCM.

  • Experience coordinating healthcare referrals, transportation, or community-based services.

Why This Role Matters

Accessing healthcare isn’t always simple. From finding the right provider to arranging transportation and understanding available resources, members often face challenges that can delay the care they need.

As a Bilingual Care Coordinator, you’ll help bridge those gaps, build trusted relationships, and make healthcare services more accessible for the individuals and communities you support.
