About the Role

Astyra is seeking a Community Outreach Coordinator to help connect individuals with healthcare providers, community organizations, and local resources throughout their assigned region.

This is a field-based position supporting members as they transition from managed care to fee-for-service healthcare coverage. A major focus of the role is helping ensure high-risk members stay connected to the care and resources they need throughout that transition.

As a Community Outreach Coordinator, you’ll build relationships with healthcare providers, community-based organizations, community health workers, and other local stakeholders. You’ll serve as a connection point between members and their communities, helping identify resources, address barriers to care, and prevent gaps in services.

This position is ideal for someone with experience as a Patient Navigator, Community Health Worker, Case Manager, Social Worker, or other community-facing healthcare professional who understands how to build trust and develop strong local relationships.

What You’ll Do

  • Build and maintain relationships with healthcare providers, community-based organizations, community health workers, and other local stakeholders.

  • Develop outreach strategies that meet members where they are and create a trusted environment for healthcare engagement.

  • Help members stay connected to healthcare and community resources during transitions in coverage and care.

  • Identify potential gaps in care and help connect members with appropriate services and resources.

  • Develop and strengthen a network of community partners throughout your assigned region.

  • Collaborate with physicians, care coordinators, community health workers, case managers, pharmacists, social workers, educators, and other partners.

  • Educate members about available community resources and advocate for their needs.

  • Help members identify appropriate healthcare and support services and access them in a timely manner.

  • Identify and proactively address barriers that may prevent members from accessing care or engaging with available services.

  • Maintain communication with members, providers, community partners, and other appropriate parties.

  • Maintain accurate and timely documentation, status updates, and required notifications.

  • Protect confidential member information and follow organizational, state, and federal privacy requirements.

  • Travel locally throughout your assigned region to meet with members, providers, and community partners.

What We’re Looking For

  • 1–3 years of community-based experience.

  • Experience as a Patient Navigator, Community Health Worker, Case Manager, Social Worker, or in a similar community-facing role.

  • Experience developing relationships with community organizations, healthcare providers, or other local stakeholders.

  • Strong relationship-building and communication skills.

  • Knowledge of community resources and the ability to connect individuals with appropriate services.

  • Cultural competency and the ability to build trust with diverse communities.

  • Strong organizational skills and attention to detail.

  • Ability to take initiative, problem-solve, and work independently in the field.

  • Ability to manage multiple priorities and changing needs.

  • Comfortable collaborating with multidisciplinary healthcare and community partners.

  • Willing and able to travel locally throughout the assigned region.

Preferred Qualifications

  • Bilingual skills.

  • Patient navigation or community health experience.

  • Case management experience or certification, including CCM.

  • Social work experience.

  • Experience working with Medicaid populations, health plans, or individuals transitioning between healthcare programs.

  • Existing knowledge of or relationships with community organizations and resources within the assigned region.

The Ideal Candidate

You’re someone who knows your community and knows how to build relationships within it. You may have worked as a Community Health Worker, Patient Navigator, Social Worker, Case Manager, or Outreach Specialist, and you’re comfortable getting out into the community rather than spending your entire day behind a desk.

You understand that access to healthcare isn’t always as simple as scheduling an appointment. Transportation, available providers, community resources, communication, and changes in healthcare coverage can all create barriers.

Most importantly, you’re comfortable bringing people together—members, providers, community organizations, and healthcare teams—to help make sure individuals stay connected to the care and resources they need.
