RN Care Manager

Responsible for the assessment, reassessment, care planning and coordination of care and services. Includes ongoing monitoring of an appropriate and effective person centered care plan, member education and care management. Regularly communicates with the members PCP and other providers, and integrates the member, caregiver and other provider feedback into the assessment and planning. Our client has an immediate need to create a new active directory to support current projects and the future more independent state when manages its own identities, credentials, policies and access controls. This position will also be responsible for supporting tasks and activities for the release/build team in the Architecture and Infrastructure Unit. This position is responsible for build activities in 27 different test, development and production environments utilized by the program. This responsibility encompasses web layer configurations, AIX, Redhat Linux/CentOS and Windows operating systems, the IBM Websphere application configurations with clustering, IBM IHS Web servers, Oracle Database and other related products.
